Franklin uses a damaged cart wheel as a metaphor for people whose faults or dissatisfaction make them unusually conspicuous. A defective wheel squeaks and rattles more than one that works properly; in the same way, a troublesome or discontented person may complain more loudly and attract more attention than those who are quietly functioning well.

The saying does not mean that every outspoken complaint is unjustified or that quiet people are necessarily more capable. Its broader point is that prominence can result from dysfunction as easily as from importance. Noise draws notice, but it is not by itself evidence that the person making it has the strongest case. Franklin therefore encourages judgement based on substance rather than on how loudly dissatisfaction is expressed.
